Your child is now legally an adult. They will need to sign a release form to allow you, the parent or guardian, access to the chart and their protected health information. If granted, this will allow us to communicate directly with you about your child’s health.
At this visit, growth is evaluated by measuring weight, height, and BMI (body mass index). In addition, nutrition and exercise counseling is offered. Mental health is assessed using the Patient Health Questionnaire for Depression (PHQ-9). Other questionnaires include screenings for cardiac, cholesterol, tuberculosis, STI, anemia and tobacco/ substance use.
Vaccines are reviewed and catch up shots are offered. An Influenza vaccine may be offered depending on availability. Vision and hearing tests are completed. A urine sample is collected in the office to screen for kidney function and infections. Fasting labs may be ordered at this visit to check for anemia, cholesterol, diabetes and STI.
